What to bring to your appointment

• Your insurance card(s)
• A Picture ID
• List of prescriptions and/or over-the-counter medications, including dose and frequency
• Information about your medical and surgical history
• Relevant records

Telephone Calls
All patients are encouraged to call with any questions they have on medical problems. Our office staff have been trained to answer many of your questions. They will also relay your information to the doctor and your call will be returned or otherwise answered at the earliest opportunity.
